Art can take many forms , a spectacle frame is only one of them. We will introduce you to new frame designers that will inspire you, that will bring out the creative artist in you.

Owner
Born and raised in Oklahoma I got my passion for eye care from my Father, as well as my love of the outdoors and sports. One of my earliest memories is going with him to see patients on a Saturday morning at Saint Anthonys Hospital before an OU game. One surprise of getting into eye care was being introduced to unique eyewear. I've been a fan of modern design encompassing a range of objects. What i've found more important is becoming familiar with the person or company behind the design, the creator of the object. That's with whom I feel I share a passion. The object, whether it is a handmade bicycle, a watch, a piece of furniture or a handmade custom spectacle frame, is the end result of that passion and love. It's satisfying to bring Oklahoman's new and unique frames and share the passion of the people behind the wonderful frames that will fill Forma. We want to become part of the fabric of downtown OKC. When I'm not working you'll find me hanging out with my son or daughter, riding my bike, driving my vintage 911 or thinking about my next trip to Italy!

Ashley Forrest is an Oklahoma City native with a background in studio arts and art therapy. Ashley is passionate about design, community, and supporting living artists. She is an avid traveler, with extensive interest in other cultures and ways of life, and a firm believer that art is a universal language. She aspires to consistently bring the most desirable local art to Forma.
Ben Blasi
Co-Owner / Optician
As an optician, the best part of my job is taking someone who views a frame as a functioning medical device and turning it into a fashion statement! I realized my passion for Eyewear when I took my first job in optics at Eyemart Express in 2005. My passion quickly turned into a career where I worked for several years at various other optical companies, until I took over the management of Physicians Optical Luxury Eyewear in 2010. It was at Physicians Optical where I took my skills to the next level by becoming ABO certified in 2011 and truly immersed myself in the world of Eyewear, that not only makes a great statement, but can define who we are as individuals. When not assisting clients find their defining “look”, I enjoy spending time with my wife Amanda, and our 3 children Lauren, Alexis and Bryce. We love riding bikes, watching movies, playing card games, and playing catch with our dog Max. I play the keyboard with our band at church and enjoy a round or 2 of tennis when time allows.
Sahara Obaid
Optician
Sahara Obaid joins Forma as our resident optician, bringing over a decade of optical and healthcare experience along with her. She fell in love with the optical world during college while working part-time as a wardrobe consultant and studying Allied Health. With its unique fusion of science, healthcare and design, it's not surprising that she found her calling in optometry. Sahara specializes in finding frames that perfectly complement your facial structure and is well-versed on the latest lens technology options to enhance your lifestyle and visual comfort. Her vibrant personality and commitment to stellar customer care provide a one-of-a-kind experience that will leave you feeling and looking smarter.

Our Roots
Forma’s roots in fact begin in 1971 in downtown Oklahoma City when Dr Jay C Johnston, established his practice at 425 NW 11th, behind what is now the Ambassador Hotel. His first Optical was The Spec Shoppe, which occupied the circular front section of what is now Stella Italian Restaurant. Now we return to a renewed and revitalized downtown, inspired to bring you the best in independent eyewear from across the globe as well as introduce you to Oklahoma’s creative types, the artists that keep this City unique and beautiful.
